Before you begin employment, your supervisor may request the following forms from you:
Training Overview
You will be participating in a comprehensive training program at Thomas Allen, Inc. The four parts of the training are:
- Standard Orientation (2 parts)
- Session 1
- Session 2
- Medication Administration
- Site Specific Orientation
Standard Orientation (both parts) and Medication Administration are presented at Thomas Allen, Inc.’s Corporate Office via lecture, video and interactive discussion. Your supervisor will schedule Site Specific Orientation with you.
Session 1
- The rate of pay for Session I is: $7.25/hour
- Dress comfortably.
What to bring to Session 1:
- Employment Eligibility (I-9) Documents
- Current automobile insurance card
- Copies of the following: CPR and First Aid certificate, CNA, PCA, Nursing, or Social Work license(s) (as applicable).
- QMRP documentation, which includes transcripts and diploma, if applicable.
- Voided check to set up direct deposit
- Bring a snack, lunch, or dinner. A pop machine is available, (60 cents per can.). A microwave and refrigerator are available.
- Current, valid Driver’s License
Session 2 & Medication Administration
- The rate of pay for Session II and Medication Administration is equal to the rate of pay for your position.
- Dress comfortably, and bring a snack, lunch, or dinner.
Site Specific Orientation
This will be scheduled between you and your supervisor. Site Specific Orientation begins as soon as you have completed Session I.
