Foster Care with Supported Living Services (MR/RC Waiver)

Supportive Living Services (SLS): Thomas Allen, Inc. currently operates thirty-three programs that provide Supportive Living Services (MR/RC Waiver) or a combination of Supportive Living Services (MR/RC Waiver) and Independent Living Services (CADI Waiver). All programs provide Foster Care Services. These services are provided in five of the metro area counties. The numbers of clients served at an individual program vary from one to four clients. These services are currently provided to adults. The persons served have a wide range of disabilities and functioning levels. Funding is provided by a variety of sources which include Medical Assistance (MA), Supplemental Security Income (SSI), Social Security, Minnesota Supplemental Aid (MSA), clients earned income (if applicable), and other benefit sources for which the client may be eligible.

CONSOLIDATED STANDARDS (MN 245B) SUPPORT TO PROVIDERS

SLS (Supportive Living Services) providers who have clients with waiver funding are required to follow certain rules and guidelines established by the MN Department of Human Services. Thomas Allen, Inc.’s role is to support these providers in maintaining compliance with these regulations. Thomas Allen, Inc. holds the host county contract and the Consolidated Standard License. Licensing is done by DHS and our providers are not directly licensed for their SLS, but sub-contracted through Thomas Allen, Inc. to provide services. The SLS providers have their own license as a foster provider. TAI provides needed assistance and ensures compliance with the Consolidated Standard Rule portion of the service delivery.

Thomas Allen, Inc. currently sub-contracts with ten individuals or groups of individuals who provide SLS Services. These 10 homes provide 28 clients with daily support. In order to help these SLS providers concentrate their attention on daily needs of the clients being served, TAI’s designated coordinators support the providers in other aspects of the Consolidated Standard requirements. Supports offered by TAI may include but are not limited to:

  • Education and oversight on the MN DHS Consolidated Standard (245B)
  • Provision of training to meet the Consolidated Standard training requirements (i.e., Vulnerable Adult, Maltreatment of Minors, Bloodborne Pathogens, Rule 40, behavior management, psychotropic medication monitoring, and other training needed to meet the specific needs of the clients served
  • Assistance in the completion of required reports, with maintaining client permanent files, outcome development and monitoring, psychotropic medication monitoring, and facilitating client meetings
  • Maintaining contractual agreements with the county, service agreements with state, and billing for services
  • Payment to the SLS provider
  • In-Home Respite Support

SLS PROVIDER’S RESPONSIBILITIES

  • Provide copy of their Foster Care License
  • Provide information for contract renewal and licensing
  • Maintain open communication regarding client’s needs for quality assurance purposes
  • Assist with development of plan and reports such as Program Abuse Prevention Plan, Risk Management Plans, Progress Reports and Incident Reports
  • Attend training sessions to fulfill the required annual training subjects and hours required
  • Comply with TAI policies and procedures
